OTS is investigating reports of a service disruption with TU Online Forms, specifically with FormAssembly forms not sending email notifications after form submission. We will post an update when we have more information available.

The issue impacting TU Online Forms has been resolved. The vendor has proceeded with a working resolution but email notifications following a form submission may still experience some delays.

The vendor is reporting an issue with DocuSign that is impacting PowerForm links. OTS is monitoring the vendor status reports and will update as information is received.

OTS is investigating reports of a service disruption with TU Online Forms.  Form Assembly webforms are causing errors, primarily with Salesforce-based Event Registrations. Engineers are working on the issue but currently there is no timeline for a resolution.  An update will be posted as information becomes available.
